Private Duty Clinical Team Manager (Home Health)
Full-Time | Mishawaka, IN / $5,000 Sign-On Bonus

Your Role
✔ Oversee patient care services and clinical staff
✔ Review and approve plans of care and physician orders
✔ Ensure timely start of care and regulatory compliance
✔ Conduct OASIS reviews and support quality improvement
✔ Coach and mentor field clinicians
✔ Partner with the Branch Director on performance metrics
✔ Support hiring and team growth

What You Bring
✔ Associate degree in nursing
✔ 2 plus years of RN experience in Home Health or Hospice preferred
✔ Supervisory experience preferred
✔ Active Indiana RN license
✔ Experience with Homecare Homebase and Microsoft Office
✔ Ability to travel within the coverage area
✔ Sit, stand, bend, lift, and move intermittently and be able to lift 50 to 100 lbs
