🩺 Your Role at a Glance
You’ll serve as the primary nurse for an assigned caseload, delivering high-quality care across the home health continuum by:
Completing start-of-care, routine, and follow-up nursing visits
Performing comprehensive patient assessments
Developing and updating individualized plans of care
Monitoring patient progress and identifying changes in condition
Communicating with physicians, clinical leaders, and care teams
Documenting care accurately and timely in the EMR
Ensuring compliance with state, federal, and agency guidelines
Maintaining patient privacy and HIPAA compliance

What We’re Looking For
Active, unrestricted RN license
At least 1 year of nursing experience in a clinical setting
Associate’s or Bachelor’s Degree in Nursing preferred
Home Health or Hospice experience preferred
Experience with HomeCare HomeBase a plus
Reliable transportation, valid driver’s license, and auto insurance
Ability to travel within the service area (approximately 50%)
Sit, stand, bend, lift and move intermittently and be able to lift 50-100 lbs.
