As a Home Health Aide (CNA) with Advocate Hospice Services, you will play a meaningful role in supporting patients and families through compassionate, skilled care. 📍 Position Details
Employment Type: Full-Time, Regular
Schedule: Monday–Friday, Day Shift, 8a-5p
Territory: South Suburban Communities (travel required)
💙 What You’ll Do
Patient Care & Safety
Provide competent, compassionate patient care as assigned
Assist patients with activities of daily living (ADLs) to promote comfort, dignity, and functional goals
Monitor patient condition and report changes to the RN/Supervisor or Case Manager
Perform personal care including bathing, feeding, linen changes, laundry, and light room cleaning
Take and record vital signs (temperature, pulse, respirations, blood pressure) as assigned
Provide skin care and assist with patient repositioning to prevent skin breakdown
Safely use equipment to ensure patient comfort and safety
Demonstrate understanding of hospice philosophy and provide calm, supportive care to terminally ill patients
Professional Practice & Communication
Work independently with reliability, professionalism, and consistent attendance
Communicate effectively with patients, families, and care team members
Participate in case conferences and Interdisciplinary Group (IDG) meetings
Complete accurate and timely documentation in accordance with agency, state, federal, and accreditation requirements
Maintain strict patient confidentiality in compliance with HIPAA and privacy standards
Quality, Compliance & Improvement
Participate actively in Continuous Quality Improvement (CQI) initiatives
Demonstrate cost-effective care practices
Assist with identifying problems and implementing solutions
Participate in quality improvement projects as assigned
Complete required in-services, hospice education hours, and OSHA training
Cooperate with supervisory visits and scheduling requirements
Provide care in accordance with all regulatory rules and standards
Teamwork & Professional Development
Work collaboratively and respectfully with all team members
Demonstrate flexibility to support departmental and service office needs
Identify personal development needs and contribute to a learning plan
Maintain and enhance Home Health Aide skills
Support team, office, company, and organizational goals
✅ What You’ll Need
High school diploma or equivalent
Minimum of one year of nurse’s aide experience
Certified Nursing Assistant (CNA) in the State of Illinois
Valid driver’s license issued by the DMV
Strong written and verbal English communication skills
Ability to care for patients across all age groups
💪 Physical Requirements
Ability to travel throughout assigned service areas in all weather conditions and on all road types, including expressways
Ability to assist with ambulation, repositioning, turning, and transferring patients (up to 200 pounds with assistance/equipment)
Ability to lift up to 35 pounds independently
Use patient‑handling equipment and team assistance for lifts over 35 pounds
Ability to work in private residences that may have limited accessibility
